Congratulations! You have been accepted into the Berkeley Youth Orchestra for its 2010-2011 season. In order for your acceptance to be confirmed, you must complete the following forms and turn them in before or at our first rehearsal and mandatory ParentsÕ Meeting on September 11, 2010, at the Laney College Theater in Oakland, held from 9 AM to 12 Noon. IMPORTANT NOTE: To qualify for BYOÕs Early Registration Discount (returning members only), you must complete and submit all registration forms and payments as described in the Tuition Contract by August 2, 2010.
Do not mail the forms in unless you are able to put the forms in the mail by September 1. If you miss the mail deadline, you MUST bring your completed forms and a tuition check or financial aid application to the first ParentsÕ Meeting. If joining after the first rehearsal, bring forms to the first rehearsal your child attends.

Print the following forms and mail to BYO
o Part Six – Signature Page
o Part Seven – Health Information and Consent for Emergency Medical Treatment
o (if applicable) Financial Aid application with first page of your 2009 1040 Tax Return
o Concurrent Enrollment Form-Peralta Community College - your school must sign this form.
o Peralta District Field Trip Permission Form

o Laney Admission form BYO members ages 12 or younger do not need to complete the Laney Admission form. All members 13 and older must complete this form. It can take 15-20 minutes to complete (Laney course info: Course Name - College Orchestra Music 30, Course Number - 41102, Department - Music, Instructor - Jay Lehmann).
The online registration process guides each family through Parts 1-7. Please remember to print Part 6 and Part 7 as these parts require signatures. Note that Javascript must be enabled in your browser to use the Print button. The Financial Aid application and Laney forms can be printed by clicking on appropriate links. When you are ready to begin online registration, click here. Registration will open in a separate window. Use the 'Next Page' button to travel though the forms until you wrap-up.
